KMYZ, “iPad A Day in May”- Official Rules 2013

1. Prizes:  One (1) 16GB Apple iPad Mini, colors may vary, valued at $329.  Up to 22 iPads to may be given away.

2. Contest open to U.S. Citizens that reside on the state of Oklahoma and are 18years of age of older.  Must have a government issued Oklahoma state photo ID as proof residency.

3.  How to enter

A) Enter to win:   Enter to win during business hours at participating sponsor locations from April 22nd – May 24th, 2013, see edgetulsa.com for a list of locations to enter. Complete an official entry form by printing your Name, Address, Phone Number(s), date of birth, e-mail address, signature and date and depositing it in the official registration box.  One entry per person, per day, per location.  Must register in person.  No mechanically reproduced entries will be accepted. Entries to be collected on or around 4/26, 5/3, 5/10, 5/17 and 5/23.  All registration boxes to be collected by Noon (12pm) on Friday , May 24th, 2013 and registration ends.

B) Mail in:On a 3”x5” piece to paper, print your first and last name, street address (No PO Boxes), city, state, zip, phone number(s) (Home and Cell preferred) and date of birth (mm/dd/yyyy) and place in a stamped envelope and mail it to KMYZ “iPad A Day Giveaway” 5810 E. Skelly Dr. Ste 801. Tulsa, OK 74135.  One entry per person, per envelope and no mechanically reproduced entries will be accepted.  You may only enter once by mail-in.  Shamrock Communications (KMYZ) is not responsible for any lost, stolen or misdirected mail.  All mail-in entries must be received by 5:30pm on Friday, May 24th, 2013.  All mail-in entries will be added to the on site entry forms as they arrive.

4.  How to win:  Up to one (1) iPad will be given away each day.  Listen to KMYZ  weekdays around 10am, 2pm and 5pm from Wednesday, May 1st through Friday, May 31st.  Starting around 10am, a random eligible name will be drawn from the iPad entry forms and announced on air.  Each name announced on air will have 10 minutes and 45 seconds to call 918- 460-1045 to claim their iPad.  If successful they win the iPad
and no more drawings will occur that day.  If person fails to call in allotted time, another name will be announced on air around 2pm.  If 2pm goes unclaimed another name will be called at 5pm.  Names can only be drawn one time, and 1 winner per household.  Odds of winning depend on number of entries.

5. Winner must claim prize within 30 days of winning. If winner(s) should fail to claim prize within 30 days, they will forfeit the prize.
 
6. Winner must claim their prize at the KMYZ studios Monday – Friday between 8:30am – 5:30pm at the KMYZ studios, 5810 E. Skelly Dr. Ste. 801 Tulsa, OK 74135.
 
7. Prizes are non-transferable and cannot be substituted for another prize, or cash at the request of the winner.
 
8. Winner agrees to sign a release for possible use of their name and likeness for promotional purposes and they indemnify and hold harmless Shamrock Communications and their participating sponsors and their respective affiliates, subsidiaries, joint venture partners, advertising and promotion agencies and the directors, officers, employees and assigns of each, harmless against any and all claims, damages, costs and liability arising out of acceptance/use of prize.  Winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize.  Sponsor and their Promotional Partners are not responsible for faulty or mis-transcribed telephone communications, dropped calls, any typographical or other errors in the printing of the offer, the announcement of any prize, in the administration of the promotion or lost or stolen entries forms or registration box(es).
 
9. Winner is responsible for all applicable taxes

10. Employees and family members of Shamrock Communications, participating sponsors, and broadcasting media outlets in the Tulsa, OK area are ineligible.
 
11. Station reserves the sole right to substitute the prize(s) for one of equal or greater value or to modify, cancel or suspend this promotion, should fraud or other causes beyond the control of Shamrock Communications corrupt the administration, security or proper conduct of the promotion.
 
12. Shamrock Communications is the sole judge of all matters pertaining to this promotion and their decision is final. All entries become property of Shamrock Communications.
